CONSENTS 同意

Note 1: If during any part of the church program, medical attention is required, an Activity Leader will contact the Emergency Contact Person, provide appropriate first aid treatment, and stay with the Participant until the Emergency Contact Person collects them. Where it is necessary the Participant is to be taken to hospital immediately, a nominated leader will accompany the Participant to the hospital.

注1：如果在教会活动期间需要医疗护理，活动负责人将联系紧急联系人，提供适当的急救治疗，并与参与者呆在一起，直到紧急联系人来接他们。如有必要立即将参与者送往医院，则由一名指定的负责人陪同参与者前往医院。

Note 2: By consenting to participating in Church of Christ Fellowship Clayton Inc Activities, you agree to indemnify and hold harmless Clayton Church of Christ Fellowship Clayton and any individual staff or voluntary leaders against all claims arising out of any injury to me (the Participant), and the relevant activity being undertaken.

Note 3: Clayton Church of Christ Fellowship Clayton may from time to time record video or take images of services and activities at the Church or at the activity location. These videos and images may be recorded, used and shown for information, ministry, teaching, fundraising and/or promotional purposes. By attending these services and activities, you agree and give consent to the Church with regard to using your video or images for such purposes.

Note 4: I understand that our English Learning program are run by volunteers who are passionate about wanting to see newcomers to Australia build English language skills, connect with others and settle well in Australia. Our program covers all the basics of listening, speaking, reading and writing in English, as well as teaching about the Australian culture, customs and practical everyday life requirements with some Christian teaching.
